I have been on the path for the Holy Grail of standby tools that will work on my system: it seems to be a very problematic one. This is what I've tried so far & why they don't work.
1) MST Standby -never worked (any of the tried versions) with the idle; as something is constantly resetting the idle time. I've tried disabling all apps & loaded programmes etc to no effect. Also, checked for possible incorrect BIOS settings; ditto. It happens around every 4-5 minutes.
2) The Hibernator -after resuming from standby the green button & the remote's extended buttons (ie. RecordedTV, Guide, LiveTV & DVD Menu) don't work. I have to do a reboot to regain functionality.
3) Shutter.exe -after resuming from an idle standby, I have no remote keyboard/mouse/joystick functionality. Again, I have to reboot to regain control. This is on two separate PCs. One has BTC9019URF keyboard. The other is using Microsoft's remote Media Center keyboard.
With regards to MST, I had/have a smiliar issue. I have my timer set to 30 mins, and at 29.55 it resets and won't go to sleep. Don't know the cause. I emailed the developer... and I'm beta testing a new version of MST (ver 0.72). It has a newer option to go to standby if no user input after xx mins.
This so far is working for me. If I'm watching TV, I'm usually hitting the skip/pause/audio controls, so that resets the timer. Same for a DVD etc. I have it set to 1 hour.
So far, working really nicely.

Unfortunately it's your hardware that is problematic. Or the drivers of that hardware.
None of the tools you mention above do anything to the machine other than tell it when to go to standby. MST does make some registry changes but it you've tried it with each option on or off through all combinations (with a reboot between each change) and you still have the problem then no amount of "standby software" is going to help.
Your problems are happening at the OS or bios level during resume. The only fix you will find is either from a bios update, identifying and replacing the drivers that are causing the issue, or replacing the hardware that causes the issue.
Unfortunately troubleshooting something like this can be a nightmare. I'd suggest pulling everything out of the machine that you can, all pci cards and any usb devices (use a normal keyboard and mouse during testing). Doing a fresh install of your OS of choice (with windows updates applied), and testing standby/resume from there. If it works then add 1 card and it's associated driver at a time and re-test. Then add back the usb devices one at a time and test. Eventually you will identify the component(s) that cause the problem.
If you don't get proper standby/resume on a base OS install with MS only drivers then it's your mainboard that will be the cause. Other than a bios update not much can be done aside from a replacement.
With regard to the not going to standby issue one common problem comes from IR keyboards. Plasma's and or the keyboard itself can send out the odd IR command that gets detected as a keystroke and keeps the machine awake. That however will not cause the resume issues that you have.
